Need to know:
Refer new child care providers to main office.
ONLY SUBMIT 2012
DO Not provide old forms 2011 to child care providers.
- Please treat providers equal and with respect.
- Please read review and home visits forms aloud during the visit, to the provider. As each item is completed make a point of telling the provider what is going on, so each review specialist format is the same and each provider knows what to expect. Follow the forms as guidance, go item-by-item listed on the review, mark off the item as it is address.
This will be build the confidence of child care providers, and increase CACFP understanding. The child care provider will better know what to expect when the State does unannounced visits, if the Children First staff uses this format.
- The home review form has addressed a few new areas:
-Record Maintenance from Oct 1, 2009 to current.
-Permanent Agreement is not renewed annually.
-Eligibility statues renewal dates
- Please place a star next to resident children on the review /ask child care providers to do the same on the menus, this is very important when tracking ratio.
-On the providers menu actually circle or mark the meal you have observed.
-Notice the 2 times a week limit of high fat and high sugar

items.
-Menu and other supplies provided: please keep ahead of
supply request, printing can be slow at times. During the








home visits, provide necessary forms.
-Actually ask providers to wash hands if they forget. The State will want to see proper hand washing.
