Where can the fingerprint clearance card expiration date be found?
The expiration date can be found on the clearance card.
New applicants and renewing applicants must follow-up with DPS after the initial prints and application have been submitted.
Department of Public Safety (DPS) 1-602-223-2279
How to Obtain a Fingerprint Clearance Card:
1.
Complete fingerprints, application for fingerprint clearance card and notarized criminal history to start the fingerprint process (forms can be obtained from Children First staff please call the main office).
2.
Send completed forms to Children First, we will be happy to submit to DPS on the behalf of Children First providers and deduct the DPS fee from meal claim check or send completed forms to DPS with a money order. DPS does not take personal checks. The time line is not guaranteed, do not wait until the last minute to return these forms.
3.
DPS will send notification directly to the child care provider stating either something needs corrected OR a fingerprint clearance card will be issued. Forward any and all notifications to Children First. (DPS can take several weeks or months to process prints the timeline is not guaranteed). Follow-up promptly on any notice DPS sends you. DO NOT WAIT for the staff to come back to your home to follow-up. It is the child care providers sole responsibility to ensure fingerprints are current.
Providers are responsible for all corrections and follow-up. In the event no notice is given from DPS the provider is still responsible for follow-up. In the event Children First does not send a reminder as a courtesy it is still the providers responsibility.
Children First only requirement in this process is to obtain a current copy all other services are done as a courtesy.
It is recommended that renewing fingerprint card applicants start the renewal process at least 4 months BEFORE the expiration date.
Be proactive take the fingerprint card and application that is provided to a local Police station, even if the fee is minimal, they are experts in this field. DO NOT WAIT for staff to come to your home and print you. Many staff members are no longer offering this service, it is not a requirement that Children First staff print providers, only that we obtain a current clearance card.
Claims will not be processed without a current valid fingerprint card. Claims processed will require re-payment of the over funds.
Sorry for the inconvenience, please take re-prints to local Police department to reprint, then send to Children First to send to DPS or send them to DPS with a money order.
